Patient Question

Hello Doctor,

I'm 17 and have genetically high cholesterol, which isn't related to my lifestyle. I haven’t seen a lipidologist yet, and I don’t plan on taking medication due to the side effects. My main concern right now is birth control. I previously tried the combination pill for my skin, but it caused severe side effects, including hormonal psychosis and suicidal thoughts. I know hormonal birth control increases the risk of stroke, and I’m already at higher risk because of my cholesterol. I also can’t get the copper IUD. Given my medical history, what are my safest birth control options?

Hello,

Thank you for reaching out to Dr. Galen. Please find the below response to your query.

1. Given your history of high cholesterol and previous adverse reactions to hormonal birth control, your best options would be non-hormonal methods.

2. Barrier methods like male condoms, female condoms, and diaphragms with spermicide are effective when used correctly. If used consistently and properly, their failure rate can be as low as 2-3%. Sponges are another option—they can be inserted a few hours before intercourse and left in place for some time afterward. However, forgetting to remove them can lead to infections, which may become serious.

3. A diaphragm may also work for you, but be aware that it requires proper cleaning and removal after use. Some users may experience latex allergies or urinary infections. Your choice should depend on your comfort, budget, and lifestyle. If you’d like more guidance, consulting a healthcare provider can help you find the best fit for your situation.

Thanks
