Patient Question

Hello Doctor,

I started feeling very dizzy. I went to urgent care and got a urinalysis. It came back high. I took 2 urinalysis strip tests at home which came back with 80 mg/dl. Is that bad?

Hello,

Thank you for reaching out to Dr. Galen. Please find the below response to your query.

The high level of ketones in your urine (5 mmol/L or above) is a serious concern and may indicate the development of diabetic ketoacidosis (DKA), a potentially life-threatening complication of diabetes. DKA occurs when there is severe insulin deficiency, causing high blood sugar levels and the buildup of ketones. Common symptoms of DKA include excessive urination, extreme thirst, fatigue, abdominal pain, nausea/vomiting, fruity-smelling breath, rapid breathing, confusion, and drowsiness. Dizziness is also frequently observed.

You should contact your doctor or diabetes management team immediately. If you feel very unwell or if your urine ketone test shows a result of 2+ or higher, you likely have DKA and need emergency medical care. DKA is treated promptly with insulin and fluids to avoid complications such as coma or even death. The elevated bilirubin and urobilinogen levels are less concerning; bilirubin may be elevated in conditions like liver disease or bile duct obstruction, and urobilinogen is a normal breakdown product of bilirubin, typically present in small amounts.

In summary, the high ketone level is an urgent medical issue that requires immediate attention. Please do not stop taking your insulin, and seek medical care right away if you experience other DKA symptoms. Monitoring ketones is crucial, especially when blood sugars are elevated or you are ill.

Thanks
